Community members in need may not always know where to seek support, but they are often interacting with medical practitioners. In partnership with local healthcare providers, Diagnosing Poverty-Prescribing Possibility asks practitioners to consider income as a risk-factor in a patient’s well-being, and refers residents to our Community Support Worker.
Clients who are referred will meet with our Community Support Workers to discuss their needs and what we can offer. There may be as few as 1 or 2 meetings, or clients may continue to meet with our Community Support Workers over a period of weeks or months while establishing a more robust support system.
